PATIENT GUIDE
Hospital admission and your stay
How to be admitted
To be admitted to the center’s hospital under the state-funded program, contact your attending doctor at the polyclinic where you are registered.
For specialized medical care, the doctor registers the patient on the Hospitalization Bureau portal and issues an admission code with the admission date.
For high-tech medical care, registration is done by the Regional Commission, which issues an admission code (quota) through the Hospitalization Bureau portal.
Documents to bring
- an identity document (ID card, passport or birth certificate);
- a referral for hospitalization stamped and signed by the head of the referring medical organization;
- a hospitalization slip with the code from the Hospitalization Bureau portal;
- a detailed extract from medical records, including laboratory and clinical test results;
- for high-tech medical care — the original referral from the Regional Commission on high-tech medical care and the Hospitalization Bureau portal code.

Patient identification rules
- On registration in the admission department, staff put an identification bracelet on the patient’s wrist showing the full name and date of birth.
- Identification is mandatory before every diagnostic or treatment procedure: medication, sampling, blood transfusion, injections, punctures, dressings and surgery.
- Make sure that before a procedure staff ask your full name and date of birth and check them against the bracelet and medical records.
- Infusion bottles and syringes must carry a sticker with the patient’s full name, date of birth, and the date and time of preparation.
- If the bracelet causes an allergy, ask for a badge instead. Do not lose the bracelet (badge) and always wear it — staff are required to check for it.
- If the bracelet is lost or damaged, the ward nurse promptly orders a duplicate; until then your name and date of birth are checked verbally against your records.
- On discharge, the ward nurse removes the bracelet.
